    Contract Agreement

    Congratulations on your acceptance into the Fitness by Example 8 Week Transformation Challenge! In order to maximize results and ensure commitment, it is essential that both parties are made clear of their responsibilities. Thus, it is necessary for both Fitness by Example and (applicant name) to agree to following terms and conditions:

    THERE ARE NO EXCEPTIONS TO ANY OF THE REQUIREMENTS BELOW

    6 am Kickstart meeting on Saturday, April 9th is mandatory! You will not allowed to be able to participate in the challenge if you do not show!

    Meeting will be held at 4500 Dike Dr., Winter Park, 32792 (Living Hope Church)

    Section I- Training Requirement

    You MUST be willing and able to provide documentation that you met with a medical advisor for a MANDATORY initial health evaluation and were cleared for participation in vigorous physical activity if requested of you.

    TRAINING SCHEDULE

    You must attend a minimum of 3 boot camp workouts per week.

    You must check in a minimum of 3 times a week on the closed group facebook page and answer the following questions:

    1. What moved me closer to my goals today?
    2. What moved me further from my goals today?
    3. What did I learn today that will help me reach my goal?

    Section II- Nutrition Requirement

    Follow the 8 week meal plan as written out and keep a food journal and must have it with you at all times. If I ask you for your journal and you do not have it, you will get 1 free chance after that you will not allowed to participate in boot camp and will count as an unexcused absence.

    Section III- Supplementation Requirement (Optional but strongly recommended)

    You have the option to purchase the recommended supplements from PROGRADE. The recommended supplements only consist of a daily multi-vitamin for your gender and a daily Essential Fatty Acid (EFA) supplement. The cost will only be $65/month. (Very important, these supplements are critically important in helping you lose the most amount of fat in only 8 weeks!) * Note: Every winner of the challenge purchased the supplements.

    Section IV- Progress Tracking Requirement

    Progress Tracking Worksheets: You MUST be willing and able to hand in weekly food journals and progress tracking worksheets as provided by Fitness by Example to assess your daily compliance to the program. You further acknowledge that if Fitness by Example is unsatisfied with your compliance they reserve the right to offer your spot to an alternate who is willing to meet these requirements.

    Body Composition Testing: You MUST be willing and able to perform the following assessments before and after each and every phase of training:
    a) Body Fat % Test
    b) Real World Test: Pant and/or Dress Size Assessment
    c) Scale Weight in lbs
    d) Any additional tests that your medical advisor mandates based on your initial health evaluation Digital Progress Tracking: You MUST be willing and able to take digital progress tracking pictures bi-weekly (in proper workout clothes and/or bathing suit clearly showing the entire body). You further acknowledge that Fitness by Example reserves the right to use these photos as testimonials for our services to be used for lead generation through various media (website, TV, video, etc.). By signing this contract, you have thus provided Fitness by Example with a media release waiver.

    Video Documentation: You MUST be willing and able to be filmed for a potential Fitness by Example Transformation Contest Documentary. This documentary may be filmed and produced by an approved video professional and you further acknowledge that Fitness by Example reserves the right to use this film for lead generation and possibly be sold as a documentary film. By signing this contract, you have thus provided Fitness by Example with a media release waiver.

    Section V- Positive Mental Attitude Requirement

    You must bring a positive mental attitude to this program. This is a once-in-a-lifetime, opportunity that will change your life forever. If you follow my proven program, I can guarantee you RESULTS. This very well may be the most challenging experience of your life but that is why it WILL be the most rewarding. I absolutely love what I do and we will not be outworked in providing you the most rapid and lasting fat loss around. I am going to be doing whatever it takes to help you realize your physical dreams, and I expect this same dedication and commitment from you as well.

    Please know that I have other applicants who will be waiting as alternates in case you are not able to complete this program. If Fitness by Example finds your effort and attitude to be unsatisfactory, I will be forced to provide this opportunity to someone else who really wants it. All I care about is helping people who actually want my help and are willing to follow my program to get results! It is a huge privilege to be accepted for this opportunity and Fitness by Example expects you to fulfill the exact requirements clearly detailed above so we can achieve your goals together. Though this may seem daunting at first, I will be there with you every step of the way to support and motivate you to get the job done, just as I am with all of my clients.

    By signing the contract below, you acknowledge all of the requirements laid out by Fitness by Example to be considered AND to remain eligible for this contest.
